Improvements and new features

1. When multiple trauma registry credentials are active, an error returned by one credential no longer causes all other credentials to pause uploads.
2. The Call Volume Visualizer now exposes its underlying data as a grid.
3. TX state sales tax is no longer required, as AngelTrack is now a Missouri corporation.
4. The Payment Event Finder has a new filter checkbox to hide those records which are not included in the Revenue Accrual Report.
5. Optimization and reduction in Google Geocoder lookups.

Significant bugfixes

1. When no affiliates are defined, the Payment Event Finder erroneously unticked the ☑ Include deleted items checkbox when filtering.
